Prerequisites

  • A MyFormCapture account - Sign up here
  • A form created in your MyFormCapture dashboard with its UUID copied
  • Joomla admin access
  • Permission to create modules

Integration Methods

Custom HTML Module (Recommended)

Create a Custom HTML module with your MyFormCapture form that can be displayed in any module position.

Copy your form UUID

Get your form UUID from MyFormCapture dashboard.
  1. Log in to your MyFormCapture dashboard
  2. Open the form you want to use
  3. Copy the form UUID (e.g. 550e8400-e29b-41d4-a716-446655440000)

Create a new module

Add a Custom HTML module in Joomla.
  1. Go to Content → Site Modules
  2. Click New
  3. Select Custom HTML
  4. Enter a title (e.g. 'MyFormCapture Contact Form')

Add form code

Paste your MyFormCapture form HTML in the editor.
  1. Click the Toggle Editor button to view HTML
  2. Replace YOUR_FORM_UUID with your actual UUID
  3. Select a Position (e.g. sidebar-right, footer)
  4. Set Status to Published
  5. Configure Menu Assignment (which pages to show on)
  6. Click Save & Close

Test

Verify submissions reach MyFormCapture.
  1. Visit a page where the module appears
  2. Fill out and submit the form
  3. Check MyFormCapture dashboard under Leads
