Add MyFormCapture forms to your Wix website using the HTML iFrame or Embed Code elements. Perfect for contact forms, lead capture, and custom submissions. Works on all Wix plans including free.

Prerequisites

  • A MyFormCapture account - Sign up here
  • A form created in your MyFormCapture dashboard with its UUID copied
  • Wix site admin access
  • A Wix site (free or premium plan)

Integration Methods

HTML Embed (Recommended)

Use Wix's HTML Embed element to add a MyFormCapture form directly to any page. Works on all Wix plans including free.

Copy your form UUID from MyFormCapture

You will need the unique UUID that MyFormCapture assigned to your form.
  1. Log in to your MyFormCapture dashboard
  2. Open the form you want to embed
  3. Copy the form UUID from the form URL or settings page (e.g. 550e8400-e29b-41d4-a716-446655440000)
Important Note
Each form has a unique UUID. Make sure you use the correct one.

Add HTML Embed element in Wix

Add an HTML element to your page where you want the form to appear.
  1. Open your Wix site in the Editor
  2. Click Add Elements (+) on the left sidebar
  3. Go to Embed Code
  4. Click HTML iframe or Embed a Widget
  5. Drag the element to your page
Important Note
Both HTML iframe and Embed a Widget work well. HTML iframe is simpler for basic forms.

Paste your form code

Configure the embed element with your MyFormCapture form HTML.
  1. Click on the embed element you just added
  2. Click Enter Code in the settings panel
  3. Select Code mode (not URL)
  4. Paste the form code below
  5. Replace YOUR_FORM_UUID with your actual form UUID
  6. Click Update

Important Note
The inline styles ensure the form looks good by default. You can adjust the embed element size and position using Wix's drag-and-drop tools.

Adjust size and publish

Resize the embed element and publish your site.
  1. Drag the corners of the embed element to resize it
  2. Position it where you want on the page
  3. Set minimum height of 400-500px for the element
  4. Click Publish in the top-right
  5. Test the form on your live site
Important Note
If the form appears cut off, increase the height of the embed element.

Verify submissions

Test the form and check your MyFormCapture dashboard.
  1. Visit your published Wix site
  2. Fill in the form and click Submit
  3. Check your MyFormCapture dashboard under Leads to confirm the submission arrived
Important Note
Submissions may take a few seconds to appear in your dashboard.
