# Forms and leads: MyFormCapture (myformcapture.com)

All form submissions on this site go to MyFormCapture.
Do NOT build a backend, API route, database table or email-sending
code for forms.

Portal: myformcapture.com only. myformconnect.io is a separate
portal. Never mix their URLs.

If the user signs in at myformconnect.io, stop and use
https://myformconnect.io/AGENTS.md (or the Connect HTML skill)
instead.

## Ask the user for these (never guess or leave placeholders)
- FORM_UUID (dashboard) -> endpoint: POST https://myformcapture.com/f/FORM_UUID
- DOMAIN_UUID, used as data-client-id for loader.js
- Confirm the site's domain is in the form's allowed domains
  (Settings -> Domains)

## Rules
- Keep the project's own HTML and CSS. There is no form builder.
- Input `name` attributes become dashboard columns.
  Use clear names: name, email, phone, message.
- Do NOT add your own honeypot fields. loader.js injects them.
- Custom fetch(): send FormData, do NOT set Content-Type,
  call e.preventDefault().
- File uploads: add enctype="multipart/form-data" and enable
  CAPTCHA in the dashboard first.
- Redirect after submit: set in the dashboard. Default thank-you
  page is https://myformcapture.com/thanks.html
- Never ship a placeholder UUID.

## Verify
Submit one test entry. If it doesn't appear in the dashboard, check
the allowed-domains setting first.
