What is a testimonial form?

A testimonial form is how you ask for a quotable sentence plus permission context. It is not a Trustpilot review widget and not a G2 listing. US B2B marketers and UK agencies both need a private intake because many customers will praise you in an email but will not click a public review site.

The request pack’s details field is the quote. Request type can mean ‘named quote’, ‘anonymous quote’, or ‘willing to do a case study call’. Preferred date is useless for testimonials—relabel it to ‘okay to publish from’ or drop the field on the form.
