Hardware vendors and fitters can take install requests on a product page. The form captures site constraints and a preferred date so project managers are not mining a generic contact inbox.

How to Use This Form Generator

1

Create a Form ID

Notify the installations coordinator.

2

Split survey vs fit

Use request types so you do not send a fitting crew to an unsurveyed site.

3

Embed on the SKU page

Pre-fill product in the details placeholder if you duplicate the form per product.

4

Confirm in your job system

This is a request until you send a date.

What is an installation request form?

An installation request is a booking-shaped lead: ‘come fit this’. That might be a heat pump, a door, a POS terminal, or an on-prem software install. US home-service brands and UK MCS installers both need access notes, storey, and whether a survey already happened.

It is not a completed job pack. Electrical certificates, gas safe records, and software runbooks happen after a human accepts the job.

Site facts that change the quote

Third-floor walk-up with no lift is not a footnote. Get it in details before you send a junior with the wrong kit.

  • Contact and phone for day-of access.
  • Request type as survey, first install, upgrade, other.
  • Preferred date plus details for address, product SKU, and constraints (parking, listed building, out-of-hours).

Do not collect alarm codes on a public form. Ask by phone after you are booked.

How installs get scheduled

Manufacturer portals, Checkatrade messages, and email all compete. HTML on your domain is the list you own.

1. HTML on the product page

‘Book installation’ under the SKU, not a footer contact form.

2. Vendor partner portals

Use when the manufacturer assigns leads. This form is for your own traffic.

3. Phone survey first

Still wise for complex sites. The form qualifies who is worth calling.

4. Self-serve calendar

Calendly works after a survey. Too early if every job needs a van pack-out.

Static installer and OEM sites

A Next.js marketing export can host the form per product line with different forms.

US permit and UK Building Regulations remain the installer’s problem. Link to your process; do not pretend the form files a permit.
