What is a school enrollment form?

A school enrollment form (UK: enrolment) is the family’s request to take up a place or to enter the admissions process. US independents often separate inquiry, application, and enrollment contract. UK independents may have registration, then a deposit. This form is the structured web step, not the legally binding parent contract your solicitor drafted.

State and maintained-school admissions in the UK usually go through the local authority portal. Do not use this HTML form as a substitute for that statutory process. It is for settings that lawfully take direct applications: independents, some academies’ supplementary forms, nurseries, and US private or charter front doors.

A first-pass enrolment, not a census

You need identity and year of entry. You do not need every vaccination date on a public marketing form.

  • Parent name, email, phone as the account you will actually call.
  • Position field as year group / grade relabelled after copy.
  • Address if catchment or boarding status matters; leave optional if it does not.
  • Experience / previous school as a short textarea, not a full transcript upload on v1.
  • Statement for SEN, sibling, or scholarship intent at a high level only.

Birth certificates, custody orders, and IEP/EHCP documents belong in a secure portal or in person—not as unrestricted public uploads.

4. LA / district portal only

Correct for many UK maintained places. Do not duplicate that process on a random HTML form.

Can we take a deposit here?

US public school registration?

Districts usually require official portals and residency proofs. This HTML form is a poor fit for that compliance load.

UK UCAS / sixth form?
