What is a pharmacy refill form?

A refill or repeat-request form is how a patient asks a pharmacy to prepare a medicine they already use. In the US this sits beside e-prescribe and insurance refill-too-soon checks; the web form is not a valid controlled-substance prescription. In the UK, NHS repeat dispensing and the NHS App are the usual channels; a private or community-pharmacy webpage form is extra convenience, not a replacement for EPS.

Use the reason field for the medicine name and strength the patient believes they need. Pharmacists still verify against the original prescription and local rules.

What a refill request should contain

You need to identify the person and the product. You do not need a scan of their passport or a photo of every box in the cupboard.

  • Patient name and date of birth to match the PMR.
  • Medicine and strength in free text (reason field).
  • Best callback number for clarifications and US counselling requirements.
  • Optional insurance or NHS number off by default unless your SOP allows it on this channel.

Never collect Social Security, National Insurance, or passport images. Never invite photos of controlled-substance bottles on a public HTML form.

How refills are requested

IVR, apps, and the NHS App exist. Small pharmacies still need a branded webpage.

1. Pharmacy-site HTML

A form on your site. Staff process a dashboard instead of a full voicemail tape.

2. NHS App / Patient Access

Correct default for many UK repeats. Keep this form for private or edge cases you advertise.

CVS and Walgreens will not run your independent brand.
