A leave of absence (LOA) is an extended step-away from work that may be paid, unpaid, protected, or discretionary. In the United States that conversation often overlaps FMLA, state family-leave laws, and short-term disability carriers. In the United Kingdom it may overlap maternity/paternity/shared parental leave, unpaid parental leave, or a career break in the contract. The HTML form is a notice to HR, not the legal determination.
Do not use this as a sick-day PTO form. Send short absences to the time-off generator so LOA exports stay meaningful.
