What is an insurance verification form?

Verification is the staff workflow that checks coverage, deductibles, and prior-auth flags before a visit. The patient-facing form is only the raw ingredients: payer name, subscriber name if different, and a callback number. US clinics live in this world daily. UK private hospitals and consultants ask for PMI (Bupa, AXA, Aviva) membership details; NHS-funded care does not use this form as a substitute for NHS eligibility.

This is not real-time eligibility software. Putting a photo of both sides of the insurance card on a public HTML form is how member IDs leak into marketing tools.

Ask for a name, not a replica of the card

Staff can photograph the card at check-in under existing HIPAA or UK GDPR controls. The website should not become a card-copy vault.

  • Patient name and date of birth matching the subscriber record.
  • Payer or PMI scheme name in the insurance field.
  • Visit or procedure to be checked in the reason field.
  • Callback number when benefits come back as ‘call us’.

Do not require Social Security, National Insurance, full member ID photos, or passport scans. Those belong in the practice-management attachment vault after in-person or portal capture.

How benefits are actually checked

Patients think a web form ‘verifies’ them. You still have to query the payer.

1. HTML pre-visit capture

2. Clearinghouses and payer portals

Availity, Change Healthcare, and UK PMI portals are where verification happens.

3. Phone to the number on the card

Still the backup when EDI returns junk.

4. Patient-portal card upload

Acceptable if that portal is in your HIPAA/GDPR boundary—not a public marketing form.

Static ‘check my coverage’ pages

Private UK consultants and US cash-friendly clinics often have no eligibility API. A static form plus a human is honest.

Say in copy that submitting does not guarantee coverage. Surprise-billing and PMI pre-authorisation rules still apply.
