Diagram match is its own row

A route can be physically open and still wrong versus the drawing tenants were shown. That mismatch is a dedicated item so it is not buried inside “obstructions.”

The item names blocked or chained exits so a Fail plus required note documents a serious finding instead of a soft “door issue” comment.
