Consultants and specialist advisors can qualify intro calls on a services page. The form captures the problem and a preferred date so you are not offering a free hour to every tire-kicker who found a Calendly link in the footer.

A consultation request is a scoped ask for a first meeting: legal, design, fundraising, architecture. US independent consultants and UK practices both use a form to learn the problem before they spend diary time. It is heavier than a generic appointment because the details field is the brief.
It is not a paid engagement. The paid work starts after a proposal. The form exists so the first call is not ‘so… what do you do?’
If you cannot tell whether you are a fit from the details field, the form is too vague.
