Atmosphere is three numbers, not one tick
Oxygen percent, LEL percent, and a toxic reading are separate numeric items so a later reader can see the actual values, not only “tested.”
Entry supervisors set space ID and times, then record atmosphere readings as numbers, ventilation, an attendant outside, briefing, communications, rescue gear, and that the permit is posted. Supervisor and attendant sign; add each entrant as another signer.
