Confined space entry permit generator

Entry supervisors set space ID and times, then record atmosphere readings as numbers, ventilation, an attendant outside, briefing, communications, rescue gear, and that the permit is posted. Supervisor and attendant sign; add each entrant as another signer.

This is an operational checklist you can embed. It is not a certified safety program or legal advice. Match items to your site rules before you rely on a signed record.

Atmosphere is three numbers, not one tick

Oxygen percent, LEL percent, and a toxic reading are separate numeric items so a later reader can see the actual values, not only “tested.”

Entrants are not a single name box

Add-signer creates a full signature block per person going in, which matches how many spaces need a list rather than one “crew” scribble.

Frequently asked questions

Does this replace a four-gas monitor download?

You type the readings. The form does not pair to a sensor via Bluetooth.

Rescue team on standby?

There is an item for retrieval equipment on site. A municipal rescue contract is outside the form.

Re-entry after a break?

Treat a new atmosphere and new submit as a new record unless your procedure says otherwise.
