Neither statute requires a public HTML form to collect a full medical history.
Use request-pack fields: name, email, request type, preferred date (event date), and details of the adjustment. Relabel request type to categories such as ‘BSL / ASL interpreter’, ‘step-free access’, ‘materials in large print’, ‘quiet space’, ‘other’.
